Additional Information
- No referral is necessary when engaging TGCA
- Typical referrals for this service are made by Hospice nurses or community based Social Workers
- We work alongside the hospice nurses and other funded agencies
- Engage TGCA to deliver as much or as little as you require
- Services can stop / re-start at any point in time as required by you
