Post Hospitalisation Home Support

The Challenge

Discharge from hospital can be planned or advised at short notice ahead of a weekend. Hospitals are keen to fast track discharge for various reasons. Returning to your home without additional support is less than ideal at a time when you want to recuperate and regain your independence.

How Can We Assist?

  • Meet you at the hospital ahead of discharge and take instructions for preparing your residence, shopping for food supplies and running errands
  • Accompany you from hospital on your way back home
  • Assist you in the early hours / days post discharge by extending trusted companionship, provide personal care, liaise with family / Doctor / Pharmacy as well as community nurses, physiotherapists and other allied health professionals.
  • We are able to put in place nursing oversight either for set time/date or on as on call.
  • Typically, a single Companion will support you to regain independence within framework of a restorative care model.
  • Your companion will be only too happy to function as an extended member of your household or a trusted friend. Your companion will do as much or as little as you require. Companions can accompany you to outpatient appointments, take you around to see friends or run errands on your behalf.

“He who has health has hope and he who has hope has everything.”

And this especially applies to the people with poor health, be it elderly age group who struggle to take care of themselves or a young post delivery mother who looks for avenues that can pamper her. This is why TGCA Care has launched its Post Hospital Discharge Support Program, a rapid response service designed to support the community.

In this competitive world with pressurizing priorities from work and home mounting upon ones being, it gets really challenging for families to take care of their loved ones. Hence, they look for support programs, which can be trusted to look after their family members that are unreachable.

TGCA Care has received numerous enquiries regarding people needing post hospital discharge assistance to either have a break from taking continuous care of the patient or due to lack of time to attend to the patient because of professional commitments. Some enquiries that are registered are listed below;

“My father has had a stroke approximately 7 weeks ago. He is now home and able to get around and has slow communication skills. He is able to go to the toilet himself and walk but tires easily and can be confused about small things. What I am enquiring about is if someone can come into the home and be with him or take home for a walk so my mum can have a break.”

“Hi I am Dean, a business executive who needs to travel a lot back and forth. My wife and I came to your hospital a week ago for IVF procedure, unfortunately given my circumstances I am unable to take care of my wife at this stage and need a professional and trained personnel who can fulfill her postdischarge needs, Giving us the services and care she requires”
